Use this form to request that a petition be sent to you by email. Be sure to follow these instructions carefully.

(1) This must be printed on "legal" sized paper, 8.5 x 14. This is not regular-sized paper.
(2) Have it printed at a local print shop. Ask them to print it as follows:
        Legal sized, double-sided, black and white, as large as will fit on the paper.
(3) Be sure all dates have month, day, and year.
(4) If their signature is illegible, ask them to also print their last name small but legible in the same signature box
(5) Be sure to include street address (no PO Boxes)
(6) It must contain City and Zip Code.
